Information for Medicare Fee-For-Service beneficiaries whose provider is participating in the Accountable Care Coalition of Quality Health II, LLC
Your provider has chosen to participate in a Medicare Shared Savings Program Accountable Care Organization (ACO). An ACO is a group of doctors, hospitals, and other health care providers who come together voluntarily to coordinate high quality care for Medicare patients, like you, to better meet your individual needs and preferences.
Your Medicare benefits stay exactly the same whether you use a physician participating in the Accountable Care Coalition of Quality Health II or not. That means there are no changes in cost or coverage. You are not being enrolled into a Medicare Advantage or Health Maintenance Organization (HMO) plan, and this does not change your Medicare Supplement coverage. You can still choose to see any doctor or go to any hospital that accepts Medicare at any time.
Your Primary Care Physician may continue to recommend that you see particular doctors for your specific health needs, but it's always your choice of what doctors you use or hospitals you visit.

You are able to select your primary clinician who you believe is most accountable for your care on MyMedicare.gov. Doing so may help your provider to engage you in health care decisions, which can improve your overall care management and health outcomes. For more information, see the beneficiary fact sheet below.
